Dear Parent/Guardian,
Due to state regulations, no medications may be given at school without a parent permission slip. In addition, we cannot give any medicine unless the medicine is sent from home with your child's name on it. We will not be giving any medications (including Tylenol, Advil, Tums, etc.) without a permission slip and your child's medicine with his/her name on it.

Please bring any medications that you wish for your child to have at school with his/her name on it and this form with your signature. If you have any questions please feel free to contact me at 776-2224.
